Contextual importance?

Sometimes in comics, a word is bolded because of "vocal emphasis," and other times its bolded because of "contextual importance." I only want to bold in the transcriptions those words that need "vocal emphasis." That means those words in which the sentence seems to mean a different thing when the word is capitalized.

Oooohhh. Contextual importance. I've always wondered why the characters seemed to yell random words at each other sometimes. I often have trouble understanding how I'm supposed to read something in the comic.

I have to say, I never would have gotten that. To me, I read the comic in the same way that people read the transcripts. If I see a bolded word, it appears to be vocal emphasis.
